About American Family Home Care
American Family Home Care was founded in May, 2005 and was formerly known as Uplink homecare. American Family Home Care provides services to people with developmental disabilities (DD), Supported Living Services (SLS) and In-Home family Support (IHFS) that is licensed under 245B-Home and Community based Waiver Services (WS). We are committed in providing residential-based rehabilitation services in Hennepin County, Minnesota for persons of any age with developmental disabilities. American Family Home Care currently has contracts with UCare, Rice County to provide services to clients with Waiver services that include CADI, CAC, MR/RC and TBI.

The staff at American Family Home Care is the heart and soul of the entire provider network. Our staff helps in monitoring and making sure that the present situation and condition of each client are taken seriously.
Our hardworking and qualified staff include:
- Registered Nurses (RN)
- Licensed Practical Nurses (LPN)
- Personal Care Attendants (PCA)
- On Call Occupational and Physical Therapists
Each of our staff is well trained in CPR, first aid, medication administration and areas specific to a client’s medical needs. We are proficient in setting up medical appointments as well as monitoring, tracking and ensuring appropriate follow ups on medical concerns.
At American Family Home Care, our client’s satisfaction is our utmost priority. That is why our staff continually updates their knowledge and training so that you get the most updated treatment and therapy sessions. Our nurses, as part of their duties, also schedule regular visits to meet patients at your convenience.
